In order to efficiently check you in at the time of your appointment, you will require:
1. Referral form from your primary physician.
Some insurance participation mandates a referral.
Please obtain an updated referral from your primary care physician in advance of your appointment.

In advance of your appointment there are certain forms that require completion to update your records.
Please download the appropriate forms prior to your visit and fill out completely. You may bring the completed forms to your office visit or fax in advance 845-339-1197
This will facilitate your appointment and reduce any potential wait-time.
