The Freedom Blue PPO enrollment kit includes an enrollment application, benefit summary, provider directory and prescription drug formulary.

Your Retiree Options

Freedom Blue PPO gives you access to all participating Blue Cross Blue Shield Medicare Advantage PPO providers and out-of-network doctors coast to coast.  Our Freedom Blue PPO network provides access to over 200,000 primary care physicians, 324,000 specialists and nearly 3,000 hospitals across the country.

Security Blue HMO (Health Options Program Legacy Plan - closed to new enrollment) gives you access to every hospital and participating provider in Western Pennsylvania. Choose your primary care physician (PCP) from thousands of participating doctors.  If you need specialty care, see any of our 11,000 network specialists.  With Security Blue HMO, you can visit the network specialist of your choice with no referral needed.  Current Health Options Program members can keep their Security Blue HMO plan. 

Both Freedom Blue PPO and Security Blue HMO plans include worldwide coverage for urgent and emergency care.

Highmark Blue Cross Blue Shield, Highmark Senior Health Company, and Highmark Choice Company are independent licensees of the Blue Cross and Blue Shield Association. Highmark Senior Health Company is a PPO plan with a Medicare contract. Highmark Choice Company is an HMO plan with a Medicare contract. Enrollment in Highmark Senior Health Company and Highmark Choice Company depends on contract renewal. This information is not a complete description of benefits.
